Patient Appointment Form

Wallace State Community College offers dental hygiene services in our modern dental clinic.  We are committed to providing patients with high quality comprehensive dental hygiene care. All treatment is provided by dental hygiene students who are supervised by licensed dental hygiene faculty and dentists. We are happy to provide the community with these services at a very low cost.  The clinic is located in the Health Science Building on campus.   

Services

  • Dental Cleanings
    • Adult New Patients and patients of record if it has been 3 or more years since last cleaning-$30.00-includes all radiographs
    • Adult Returning Patient-$30.00-includes all radiographs
    • Children New and Returning Patient under 17 - At no cost - Our partnership with the Alabama Department of Public Health allows for treatment at no cost for children and expectant mothers.
  • Sealants-$6.00 per tooth
  • Bleaching-$100.00.  Patients must have their teeth cleaned in our clinic before receiving bleaching and must be approved for bleaching by our faculty Dentist.

Payment is required at the initial visit. We do not accept dental insurance. We accept cash, check and credit cards.

Hours of Operation

Clinic days vary depending upon the semester. Appointment times are from 8:00-10:30 and 12:00-2:30.  Please call 256-352-8300 to schedule an appointment.

Clinic Policies

This is a teaching clinic. The care provided by our student hygienists is done in such a way that gives priority to education.  This may result in longer treatment appointments than would occur in a private dental practice. Please expect your appointment to last at least 2 hours.  In some cases you will need to return for additional appointments.
 
The students depend on patients to be available for their clinic sessions. Appointments should be cancelled only if absolutely necessary and at least 24 hours in advance. Failure to show for your appointment twice in one semester may result in you being asked to seek treatment in private dental practice. 

Payment is expected when services are provided. We accept cash, checks, debit and credit cards.

Please bring a list of medications to your initial appointment.

Patients over the age of 18 will receive a screening appointment prior to a cleaning appointment. The purpose of the screening is to determine the oral health condition and take any necessary radiographs. Every attempt is made to begin the cleaning after the screening, but this is not always possible.  You may have to return for an additional appointment for your cleaning.

We do not provide any dental services (fillings, extractions, etc.) Our faculty dentist will provide a referral form for you to take to your private dentist if you have any dental treatment that is needed. We will email radiographs to your dentist upon request.

Wallace State Community College Dental Hygiene Clinic must abide by guidelines outlined by the Occupational Health and Safety Organization (OSHA) and by the Commission of Dental Accreditation. It is not appropriate, because of potential biohazards and for safety reasons, for any nonprofessional person other than the patient to be in the clinical area. This includes children who accompany patients to the clinic. Unattended children are not allowed in the reception area.  Persons who are not patients are not allowed on the clinic floor. This requirement may be waived if there are extenuating circumstances discussed with and approved in advance by clinic staff.

Food and drink are not allowed in the clinic area. Cell phones should not be used in clinic and should be set on vibrate. We appreciate your assistance to ensure that this policy is followed.
